From the site of Ukrainian Heraldry:
"It was confirmed in December, 19th, 2003 by the decision #25 of IV regional council session of X convocation. Rectangular panel with sides ratio as 2:3 has three equal horizontal stips - green, yellow and blue, from staff edge goes white vertical strip in 1/4 of flag length, on it are three black fancy flowers."

"It was confirmed in December, 19th, 2003 by the decision #25 of IV regional council session of X convocation. Vert; azure wavy base is charged by three argent wavy fillet fesses, on or bank is or wooden wall with opened azure gate, in it are three or crossed arrows, in the argent chief are three sable fancy flowers. The shield crowned by or crown. Below shield goes the azure tape with or inscription "Khmel'nyts'kyi region". The authors are V.Il'ins'kyi, A.Grechylo, V.Napytkin."
