The canton was formed on 25 June 1824.
Description of the symbols, from the municipal website.
"The flag of Otavalo has the following characteristics: Blue, red and green in horizontal stripes placed in this order to reproduce the nature's order: on top, the always blue sky, on bottom, the emerald fields always in spring, separated by a red stripe, symbol of the original nobleness of the place and of the ardor of the blood of the inhabitants, always prepared to realize great and generous actions. The coat of arms of Otavalo is placed in the middle"
The photograph placed beside the text shows a flag with golden writing in the upper and lower stripes.
"The reconstructed coat of arms is gules a castle or with one tower, standing on green mounts, between which appears, dexter, volcano Imbabura [an inactive stratovolcano (4,609 m asl) not thought to be entirely extinct. See: wikipedia. I.S.] In base, a lagoon argent and azure represents the lagoon San Pablo [located 4 km west of the town of Otavalo, 2;670 m asl. Locally known as Imbakucha. See here. I.S.].
The orle azure is charged with seven castles argent representing the seven villages forming once the Corregimiento of Otavalo [officially formed in 1563. I.S.]: Cotacachi, Atuntaqui, Urcuquí, San Pablo, Tabacundo, Cayambe and Cangagua. The shield is topped by a mural crown, recalling the grant of the title of city by Simón Bolívar on 31 October 1829."  
The flag on the image above, slightly differs from the flag on the aforementioned photograph (skipping the writing): on the photograph, the upper edge of the coat of arms is placed on the border of the upper and middle stripes of the flag, while it overlaps the blue stripe on the drawing.
