Overview

Pachavita is a municipality in the Colombian Department of Boyacá, part of the the subregion of the Neira Province. It was founded on 17 November 1796.
The flag of Pachavita is 2:3 in ratio, vertically divided green-white-green and is charged with a tree ("chizo"). According to the description, the tree should have nine branches for the nine "veredas" of this municipality, but it isn't seen as such in the image.
The flag was adopted by the municipal council's decree No. 012 of 24 November 1982 (full text of the decree is in the municipal web site) and it was designed by José Luis Huertas.

Relevant parts of the Decree (translated, skipping the administrative prescriptions):
"Article 2.
The dimensions and colours shall be as follows:
Stripes: The flag shall be made of three vertical stripes: The first, vert (green) shall cover one third of the flag, the second, central, white shall also cover one third of the flag. The third remaining part shall also be made of vert (green).
a) In the middle of the white part shall be placed the design of a tree, representing a "chizo"*, the emblematic tree of our municipality and from the main trunk of which radiate nine branches representing the nine rural hamlets ("veredas")** prescribed by the territorial division.
b) The length of the flag shall be the vertical dimension added with a third part and the same dimensions as for the national flag shall be adopted.
Colours:
a) Argent (white), means the requirement to protect the orphans and their future to preserve peace [...]
b) Vert (green), means the requirement to help the workers and peasants who demonstrate faith, sincere friendship, service, repect and hope in better times, outlined by the beauty and fertility of their fields.
Article 3. It is recommended to the administration, educational bodies and other bodies of the community to hoist the municipal flag on all national and local holidays."
José Luis Huertas signed ("firmado") the Decrees prescribing the flag and coat of arms, but is not explicitely listed as the designer of the symbols.
* "chizo", a.k.a "chizo zapote", is the sapodilla, Manilkara zapota.
** Aguaquiña, Buenavista, Centro, Guacal, Hatogrande, Llanogrande, Pie de Peña, Sacaneca and Soaquira.
